CORONAVIRUS


IF YOU SUSPECT YOU MAY HAVE, CORONAVIRUS SYMPTOMS EG: A NEW CONTINUOUS COUGH or a TEMPERATURE OF 37.8 C OR HIGHER, PLEASE DO NOT ATTEND SURGERY, YOU SHOULD ISOLATE THE SICK FAMILY MEMBER AND ANY ONE ELSE IN THE HOUSEHOLD FOR 14 DAYS AND IF CONCERNED ABOUT YOUR SYMPTOMS  USE THE NHS 111 ONLINE SERVICE,IF YOU CAN NOT GO ONLINE YOU CAN CALL 111 FOR ADVICE AND GUIDANCE, BUT YOU MAY HAVE A CONSIDERABLE WAIT.  THOSE OVER 70, PREGNANT WOMEN AND PATIENTS WITH SOME LONG TERM CONDITIONS ARE BEING ADVISED TO SELF ISOLATE OR TAKE ACTION TO SOCIALLY DISTANCE THEMSELVES FOR, A YET TO BE DETERMINED, LENGTH OF TIME.

If you live in our catchment area and would like to join the practice please complete a registration form which can be picked up from reception along with an information pack.

Please bring the completed form back along to reception with your medical card if you have it. Don’t worry if you can’t find it, we can give you a short additional form requesting details of your previous surgery. We also need proof of identity and address (e.g. passport, utility bill).

Once you have been accepted as a patient your medical records will be transferred to us.  We may ask you to attend  a new patient health check to review your health and medications. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

Medical treatment is available from the date of registration. Please contact reception for further information.
