If, when we take your blood pressure at the surgery, your reading is higher than is desirable we may ask you to supply us with blood pressure readings that you yourself take at home. This will assist the practice clinicians to calculate your average blood pressure. If a clinician asks you to purchase a blood pressure monitor from a local pharmacy you can then print off and complete the form below returning it to us, you will then be informed if you require further management of your blood pressure.
